Building Healthier Communities with Community Hubs

Creating healthier communities requires more than good intentions—it takes collaboration, innovation, and a willingness to rethink how we approach chronic disease. That was the central theme of the recent Bite Sized Bits Community Hub Webinar, sponsored by the Advancing Healthy Lifestyles Coalition, where leaders from public health, community organizations, and healthcare came together to share a bold new approach to tackling obesity and related conditions.

A Shift in How We View Obesity

One of the most important takeaways from the discussion is a growing shift in perspective: obesity is not simply a lifestyle issue—it is a complex, chronic disease linked to over 200 health conditions, including type 2 diabetes, heart disease, and kidney disease. With projections showing that nearly half of U.S. adults may be living with obesity by 2030, the urgency to act is clear.

Tanya Haley with the American Diabetes Association and Shamar Harrison with the YMCA of Delaware emphasized the need to move beyond stigma and toward evidence-based care. That includes improving how healthcare providers communicate with patients, addressing bias, and creating more supportive, personalized treatment approaches.

The Power of Community-Based Solutions

At the heart of the webinar was the introduction of the Community Hub, a model designed to connect individuals with the resources they need to live healthier lives.

Rather than functioning as a clinic, the Community Hub serves as a central access point—linking people to services such as:

  • Food and nutrition support

  • Housing and transportation assistance

  • Medical care and chronic disease programs

  • Employment and financial resources

Located at the Central YMCA in Wilmington, with expansion into Sussex County, the initiative brings together over 35 referral partners to address the social determinants of health—the real-life factors that often shape health outcomes.

Breaking Down Barriers to Care

A recurring theme throughout the webinar was access. Many individuals face barriers that go far beyond healthcare—lack of transportation, food insecurity, or inability to afford medications.

The Community Hub tackles this head-on by:

  • Offering walk-in support and easy referral pathways

  • Providing one-on-one assistance through community health workers

  • Hosting educational sessions on topics like nutrition, mental health, and chronic disease

  • Organizing community vendor fairs with direct access to services

Importantly, participation is free and open to all—no YMCA membership required.

Collaboration Is the Catalyst

This initiative is not a standalone effort. It represents a powerful partnership between organizations like the American Diabetes Association, YMCA of Delaware, and local community coalitions.

By creating a shared referral system, the program breaks down silos and ensures that individuals don’t fall through the cracks. Whether someone enters through a community event, a healthcare provider, or a simple walk-in, they are guided to the support they need.

Looking Ahead

The Community Hub is still in its early stages, but early results are promising, with dozens of individuals already connected to critical resources. As the program grows, the goal is to expand across Delaware and continue refining a model that can be replicated in other communities.

The message from the webinar was clear: improving public health requires more than treating symptoms—it requires addressing the full picture of people’s lives.

And through initiatives like the Community Hub, that vision is starting to become a reality.
